Hospital to Home Care Package After a long hospital stay, returning home can feel overwhelming—both for you and your loved ones. Our Hospital to Home Care Package is designed to help you regain confidence in your own space, ensuring a smooth transition and offering essential support in the first 48 hours after discharge.

What We Offer:

Pre-Discharge Meeting: We’ll meet you in the hospital before your discharge to get to know you, listen to your needs, and help prepare your home. If possible, we encourage your family to join these conversations, and we will coordinate with hospital staff to ensure a seamless discharge process.

Transport Home:

Where possible, we can provide transportation to get you safely from the hospital to your home.

Settling In:

Upon arrival at home, we’ll help you settle in. We'll unpack your bags, start any necessary laundry, confirm your medications are correct, and ensure you have something to eat and drink.

48-Hour Wraparound Care:

Over the next two days, we’ll visit you up to four times per day, including morning and bedtime support. You’ll also receive three meals a day, with assistance where needed.

Ongoing Support:
